Important information to consider when making a report

The purpose of the investigation process resulting from a report of illegal practice is not to analyze the quality of the reported individual's practice. Its purpose is to determine whether that person is practicing a reserved activity when he or she is not authorized to do so.

If you have questions about the practice of a member of a professional order, contact the Office of the Syndic of his or her professional order.

Information to be included in the report

The form allows you to identify the person or organization that is the subject of the report as well as the activities that you believe are being carried out illegally. It may also concern a usurped professional title. The following information is required:

  • The person's first and last name
  • Name of the organization
  • Complete contact information (address, postal/zip code, city, province/state, country, phone number and e-mail address)
  • Link to website(s)
  • Name of employer, if applicable
  • Title that appears to have been usurped (e.g. usurpation of the title of psychologist or psychotherapist) and the nature of the reserved activities that you believe are being illegally practiced
  • Documents or evidence of the illegal practice (business card, photo of advertisement, screenshot of Facebook ad, informational flyer, etc.)
